Patient Care Specialist

We are seeking a skilled Patient Care Specialist to join our interdisciplinary team at RehabCare. As a key member of our care team, you will play a vital role in providing exceptional patient care and supporting the recovery journey of individuals with complex medical needs.
About the Role:
- Provide personalized patient care services, including assistance with daily living activities, mobility, and hygiene.
- Support patients with meal preparation, serving, and removal of food trays, as well as assist with feeding or preparing meals as needed.
- Respond to patient call lights, anticipate patient needs, and make regular rounds to assigned patients.
- Assist patients with handling personal property, such as dentures, glasses, contact lenses, hearing aids, prosthetic devices, etc.
- Report and record any changes observed in patient condition or behavior, as well as unusual incidents.
- Collaborate with licensed nursing staff to support treatment procedures and provide optimal care outcomes.
Requirements:
- Certified Nursing Assistant (CNA) certification or equivalent experience in an acute or long-term facility.
- Current CPR/BCLS certification.
- Minimum six months of experience in a healthcare setting preferred.
- Excellent communication and interpersonal skills.
