Job Summary

Turning Point Community Programs is seeking a skilled Behavioral Health Specialist II to join our Transitional Support Services (TSS) South program in Sacramento. As a key member of our team, you will provide psychosocial, rehabilitation, and recovery-oriented services to our clients, empowering them to achieve their goals and improve their overall well-being.

Key Responsibilities
  • Provide individualized support and advocacy to clients, helping them access and receive mental health treatment, medical care, housing, food, and clothing as needed.
  • Develop and implement individual service plans, ensuring clients receive holistic care and support.
  • Collaborate with clients, families, and care providers to ensure seamless care coordination and communication.
  • Provide on-the-spot counseling and coaching to clients, promoting age-appropriate social skills, money management, personal hygiene, and job readiness.
  • Assist clients in establishing eligibility for Medi-Cal or other benefits and advocate for continuation of benefits when necessary.
  • Develop financial management plans for clients receiving Representative Payee Services and authorize expenditures as needed.
  • Interact productively with the team to provide holistic case management services to an identified group of clients.
Requirements

To be successful in this role, you will need:

  • A bachelor's degree in Social Work or a related field.
  • Four years of experience as a provider of direct mental health services or meeting other Mental Health Rehabilitation Specialist qualifications.
  • A California driver's license and current vehicle insurance/registration.
  • Reliable means of transportation capable of passing a vehicle safety inspection.
