Occupational Therapist Home Health Specialist

We are seeking a skilled Occupational Therapist to join our Home Health team. As a key member of our interdisciplinary team, you will play a vital role in helping patients regain their independence and improve their quality of life.
Responsibilities- Assess and evaluate patients' daily living and work-related skills, developing personalized therapeutic retraining programs to achieve measurable objectives.
- Administer and interpret diagnostic and prognostic tests to assist physicians in evaluating patients' functional abilities.
- Collaborate with healthcare team members to develop and revise patient care plans, ensuring comprehensive and coordinated care.
- Provide evidence-based occupational therapy interventions to improve patients' strength, coordination, range of motion, and function, or teach compensation measures as needed.
- Review and expand retraining programs to align with patients' behavioral goals and rehabilitation program objectives.
- Consult with vocational team members to develop vocationally oriented plans, ensuring patients receive holistic care.
- Communicate patient status and progress to physicians, rehabilitation staff, clinical managers, patients, and family members.
- Participate in care coordination and discharge planning activities, serving as a resource for other healthcare personnel.
- Design community reintegration activities to support patients' physical reconditioning and psychological adjustment, coordinating plans with interdisciplinary team members.
- Recommend and design special adaptive equipment to enhance patients' living or working environments, or improve their participation in the rehabilitation program.
- Train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills, and participate in team and family educational meetings.
- Supervise, instruct, and evaluate Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
- Accurately document patients' care observations, interventions, and evaluations, ensuring timely submission of evaluation, treatment plans, and discharge summaries.
- Master's degree from an accredited Occupational Therapy program.
- A minimum of six months of occupational therapy experience, with Home Health experience preferred.
- Current and unrestricted OT licensure.
- Current CPR certification.
- Good organizational and communication skills.
- Valid driver's license, auto insurance, and reliable transportation.
