Key Responsibilities
  • Provide direct patient care according to the provider's prescribed plan of care and nursing scope of practice
  • Conduct skilled assessments and develop comprehensive care plans that address patient needs
  • Communicate patient conditions and collaborate with healthcare providers to deliver seamless care
  • Complete accurate and timely clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
  • Provide education and support to patients and their families to promote preventative care and continuity of care
Requirements
  • Graduate of a practical (vocational) nursing program for LPN/LVN and current licensure in the state(s) of practice
  • At least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program
  • Current CPR/AED/BLS/First Aid certification
  • Reliable transportation to/from care sites and/or work locations
  • Two (2) years of experience practicing as a Licensed Practical Nurse (LPN) in a home health or similar setting
