Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse to join our team at The Sheridan at Eastside. As a key member of our healthcare team, you will provide high-quality nursing care to our residents, ensuring their physical, emotional, and social needs are met.

Responsibilities
  • Administer medications to residents in accordance with Federal and State regulations and Senior Lifestyle Corporation guidelines.
  • Conduct admissions assessments and documentation for new residents.
  • Develop and implement acute and long-term care plans or service plans.
  • Track clinical systems, such as wounds or infection control, under the direction of the director.
  • Provide general consultation on health-related concerns within the community.
  • Notify physicians and/or family members of any changes in a resident's health and document accordingly.
  • Respond promptly to emergencies and maintain close communication with all departments and staff members.
  • Provide educational meetings for residents and staff.
  • Maintain confidentiality of residents' and prospective residents' information.
  • Assist with discharges, assessments, and clinical documentation as needed.
Requirements
  • Training from an accredited licensed or vocational nursing program.
  • 1+ years of experience in senior care or a related field.
  • Current Licensed Practical Nurse's License in the state of practice.
  • CPR Certification.
  • Acceptable proficiency in English to perform job duties, communicate with supervisor and other employees, and respond to emergency situations.
  • Basic computer skills.
