Home Health Nurse Opportunity

St. Luke's Health System is seeking a dedicated and compassionate Registered Nurse to join our Home Health team in Boise, Idaho.

About the Role

We are looking for a skilled and empathetic nurse to provide high-quality care to patients in the comfort of their own homes. As a Home Health Nurse, you will work closely with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient.

Responsibilities
  • Provide comprehensive nursing care to patients in their homes, including wound care, medication management, and education on disease management.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop and implement plans to address these needs.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless care coordination.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with care plans.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements
  • Current RN license in the State of Idaho.
  • 1 year of experience in home health or acute care nursing.
  • Basic Life Support (BLS) certification.
  • Valid driver's license and proof of auto insurance.
