About the Role: As a Clinical Care Specialist, you will work closely with our skilled nursing facilities, assisted living facilities, and memory care communities to provide comprehensive support services to residents. Your expertise in clinical social work will enable you to develop effective care plans, lead multidisciplinary teams, and ensure seamless transitions for patients.

Responsibilities:

  • Lead and manage interdisciplinary teams to develop and implement individualized care plans.
  • Provide direct care and counseling services to residents, focusing on mental health support and disease management.
  • Collaborate with healthcare providers to coordinate medical services, including medication management and treatment planning.
  • Maintain accurate records and reports of patient progress, ensuring timely communication with stakeholders.
  • Promote positive relationships between residents, families, and healthcare providers through open communication and education.

Requirements: To succeed in this role, you must be a Licensed Clinical Social Worker (LCSW) with a strong background in gerontology, mental health, or a related field. We offer competitive compensation, commensurate with experience, which ranges from $120,000 to over $150,000 annually.
