Job Summary:

We are seeking a highly skilled Physician Assistant to join our team at Rehabilitation Associates. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional care to our patients in short-term rehabilitation facilities, long-term care facilities, and sub-acute care facilities.

Key Responsibilities:

  • Conduct Comprehensive Patient Examinations: Perform thorough examinations, obtain medical histories, and assess the health status of patients to develop effective treatment plans.
  • Diagnostic and Therapeutic Services: Order, perform, and interpret diagnostic tests; prescribe medications and manage treatment plans to ensure optimal patient outcomes.
  • Consultation and Referrals: Collaborate with physicians on complex cases and coordinate specialty referrals to provide comprehensive care.
  • Patient and Family Counseling: Provide guidance on preventive health care and manage patient and family education to enhance understanding of treatment plans.
  • Triage and Patient Communication: Handle patient calls, evaluate concerns, and triage effectively to ensure timely medical responses.
  • Documentation and Reporting: Maintain comprehensive medical records and develop status reports to track patient progress.
  • Interdisciplinary Collaboration: Actively participate in team meetings to discuss patient care, treatment modifications, and progress.
  • Facility Metrics Meetings: Engage in monthly meetings to discuss key facility metrics, including rehospitalization rates and acute care transfer rates, to improve facility outcomes and patient care.

Requirements:

  • Active Physician Assistant License
  • Minimum two years of experience as a PA, preferably in an acute, subacute, or rehabilitation setting
