Position Overview:
The Care Coordination Specialist II plays a vital role in supporting administrative functions within care management. This position involves conducting outreach, managing incoming communications, and organizing service appointments.
Key Responsibilities:
1. Act as a primary contact for members, healthcare providers, and internal teams to address inquiries and resolve issues.
2. Maintain accurate documentation of member interactions in compliance with state regulations and organizational policies.
3. Facilitate outreach efforts to members via telephone, assisting them with care plan implementation, resource navigation, and addressing any questions or concerns.
4. Provide support to members in connecting with health plan and community resources to ensure optimal service delivery.
5. Utilize knowledge of health plan activities to assist members effectively.
6. Handle various inquiries from members and providers, including clarifying care plan procedures and protocols.
7. Support member onboarding processes and manage day-to-day administrative tasks, including correspondence and educational materials.
8. Document and maintain non-clinical member records to uphold standards of practice and regulatory compliance.
9. Identify and refer members to resources addressing Social Determinants of Health (SDOH) needs.
10. Perform additional duties as required.
Qualifications:
- High School diploma or GED is required.
- A minimum of 2 years of relevant experience in a similar role.
