RN - Medical/Surgical Transitional Care Specialist

We are seeking a skilled and compassionate Registered Nurse (RN) to join our Transitional Care Unit at Kaiser Permanente. As a key member of our healthcare team, you will provide high-quality patient care and coordinate with other disciplines to ensure seamless transitions for our patients.
Responsibilities- Assessment: Conduct thorough, multi-system assessments to identify patient needs and develop individualized care plans.
- Planning: Collaborate with patients, families, and healthcare teams to develop and implement comprehensive care plans that address physical, emotional, and educational needs.
- Implementation: Deliver evidence-based care, administer medications, and perform procedures as needed, while maintaining accurate documentation and communication with the healthcare team.
- Evaluation: Continuously evaluate patient progress, adjust care plans as needed, and participate in quality improvement initiatives to enhance patient outcomes.
- Minimum 9 months of RN experience, with 1 year of current telemetry inpatient experience.
- Graduate of an accredited nursing program.
- Registered Nurse License (in the state where care is provided).
- Basic Life Support and NIH Stroke Scale Certificate within 3 months of hire.
- Advanced Cardiac Life Support.
- Previous healthcare experience.
- Bachelor's degree in nursing or related field.
- 1 year of current experience in progressive or intermediate care unit.
- Recent experience with respiratory compromised patients.
- PCCN/CCRN Certification.
This is a part-time position, working 24 hours per week, with a night shift schedule. The work location is on-site at Kaiser Sunnyside Medical Center in Clackamas, Oregon.
