Clinical Pharmacist Job Description

Job Summary:

Clinical Pharmacists play a vital role in optimizing safe and effective drug therapy in various clinical settings. As a Clinical Pharmacist at the University of Maryland Medical System, you will be responsible for providing clinical and operational pharmacy services to ensure the best possible patient outcomes.

Clinical Responsibilities

Direct Patient Care:

You will participate in direct patient care and interdisciplinary rounds, interacting with physicians, nurses, and other healthcare professionals to guide and recommend appropriate medication use.

Focused Clinical Initiatives:

You will participate in focused clinical initiatives, such as pharmacokinetic consultation, IV to PO conversion, renal dose adjustment, and other department-defined projects.

Therapeutic Treatment:

You will demonstrate knowledge and skills to recommend appropriate therapeutic treatment for neonatal, adult, pediatric, adolescent, and elderly patients.

Medication Monitoring:

You will monitor assigned patient areas for clinical progress and evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, recommending adjustments as necessary.

Documentation and Education:

You will document clinical activities and interventions on a regular basis and provide drug information and education to other pharmacy staff and healthcare professionals.

Leadership and Collaboration:

You will serve as a primary preceptor for pharmacy residents and students, assist with departmental orientation, and participate in departmental quality improvement processes.

Medication Distribution

Dispensing and Storage:

You will understand and follow departmental and hospital policies related to dispensing of medications, verify medication orders, and dispense medications accurately, safely, and in a timely manner.

Quality Assurance and Security:

You will maintain proper storage of all pharmacy products, oversee the filling of all stock requisitions, and ensure the security of controlled substances.

Problem-Solving and Technology:

You will independently resolve problem medication orders, process and validate medication orders using current information system technology, and maintain proficiency in hospital computer systems and pharmacy automation technology.

Leadership and Collaboration:

You will direct and supervise the work of technicians, complete unit inspections, and coordinate activities with other departments to provide comprehensive services.

Qualifications

Education and Experience:

You must have a Doctor of Pharmacy degree, a current Maryland license in good standing as a pharmacist, and a minimum of 2 years of experience as a pharmacist in a hospital with clinical pharmacy responsibilities.

Additional Training and Certification:

Additional training or certification, such as board certification in an area of practice, is desirable.
