- Coordinate and facilitate the development of individualized care plans for patients receiving hospice services.">
- Collaborate with healthcare teams to ensure seamless delivery of care and effective communication among multidisciplinary members.">
- Provide skilled nursing care and perform tasks such as vital sign assessment, wound care, IV management, and ostomy care within established plan of care guidelines.">
- Evaluate patient progress and adjust care plans as necessary to meet changing needs and goals.">
- Document visit notes, treatments, and care according to regulatory requirements and organizational standards.">
Required Skills and Qualifications
">To be successful in this role, you will need:

- A current Registered Nurse (RN) license issued by a compact state or the state where you reside, depending on the location of the position.">
- Basic Life Support (BLS) certification from the American Heart Association (AHA).">
- Reliable transportation and a valid driver's license, as well as a clean driving record and adequate insurance coverage.">
- Able to work independently and collaboratively as part of an interdisciplinary team.">
- Excellent communication and interpersonal skills, with the ability to effectively communicate with patients, families, and healthcare professionals.">
