Bridge Home Health and Hospice is seeking a compassionate and skilled Certified Home Health Aide to join our team. As a Home Health Aide, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.
Key Responsibilities
- Provide personal care services to patients, including bathing, grooming, and toileting assistance.
- Assist patients with daily living activities, such as dressing, feeding, and transferring.
- Monitor patients' conditions and report any changes to the assigned nurse.
- Document patient care and maintain accurate records.
- Collaborate with the healthcare team to develop and implement patient care plans.
- Perform light housekeeping and laundry duties as needed.
- Assist patients with medication management and administration.
Requirements
- Current certification as a Home Health Aide.
- High school diploma or equivalent required.
- Ability to read and follow written instructions.
- Self-directed with the ability to work with minimal supervision.
- Empathy and compassion for patients and their families.
- Current CPR certification.
