We are seeking a dedicated and compassionate Registered Nurse to join our team as a Nurse Care Coordinator. In this role, you will be responsible for creating and managing patient care plans, coordinating with colleagues to assess, plan, implement, or evaluate patient care plans, administering medications, monitoring patients for side effects or reactions, documenting any changes in patient conditions, and reporting on changes.

Job Responsibilities:

  • Coordinate with colleagues to assess, plan, implement, or evaluate patient care plans
  • Administer medications and monitor patients for side effects or reactions
  • Document any changes in patient conditions and report on changes
  • Maintain accurate records of patient care and discuss observations with the supervisor

Qualifications:

  • Currently licensed Registered Nurse (RN) in state of employment
  • Previous experience as a Registered Nurse is preferred
  • Excellent interpersonal and client care skills
  • Valid driver's license
  • First aid/CPR certification required
