About the Role

We are seeking a highly skilled and compassionate Senior Patient Care Manager to join our team at Heartland Hospice. As a key member of our leadership team, you will be responsible for overseeing the day-to-day operations of our hospice sites, ensuring the delivery of high-quality patient care, and leading a team of dedicated professionals.

Key Responsibilities
  • Supervise and manage the performance of team members, ensuring they are equipped to provide exceptional patient care.
  • Develop and implement strategies to improve patient outcomes, staff engagement, and operational efficiency.
  • Collaborate with the Executive Director and Administrator to drive business growth, improve patient satisfaction, and enhance the overall patient experience.
  • Manage and oversee clinical and administrative functions, ensuring compliance with regulatory requirements and company policies.
  • Provide leadership and guidance to team members, promoting a culture of excellence, empathy, and compassion.
Requirements
  • Registered Nurse with a degree in nursing and a minimum of one year of experience as an RN.
  • Three years of experience in direct patient care, with a minimum of one year in a leadership or management role.
  • Proven track record of improving patient outcomes, staff engagement, and operational efficiency.
  • Excellent communication, leadership, and interpersonal skills.
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple projects simultaneously.
