Case Manager

Terros Health is seeking a highly motivated and compassionate Case Manager to join our team at our 51st Ave Healthcare Clinic in Phoenix, AZ.
The ideal candidate will be a self-starter, organized, and professional individual who is passionate about providing supportive services to individuals diagnosed with Serious Mental Illness.
As a Case Manager, you will work collaboratively with an interdisciplinary clinical team to engage, educate, communicate, and coordinate care with members, their families, behavioral health, medical, and dental providers, community resources, and others.
Key Responsibilities:
- Assist clients with identifying treatment and recovery goals and provide supportive services to help clients reach their goals.
- Complete assessments, crisis plans, and individualized service plans.
- Work collaboratively with the clinical team to engage, educate, communicate, and coordinate care with member, their family, behavioral health, medical, and dental providers, community resources, and others.
- Conduct outreach and follow-up of services, including crisis interaction, missed appointments, and hospital discharges.
- Perform all case management functions associated with caseload, including completing the assessment and service planning processes.

Qualifications:
- Education/Experience:
- GED/High school diploma with a minimum of 1.5 years of behavioral health experience or an associate's degree in the field of medical or behavioral health, or a bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience, or a bachelor's degree in the field of behavioral health, or a master's degree or higher in the field of behavioral health.
- Computer Skills: Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferred.
- Valid AZ Driver License.
- Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment.
- Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.
- Good communication skills, evidenced by a non-judgmental approach, effective listening, and positive interactions.
- Basic understanding of the DSM-V preferred.
- Certified Case Manager strongly preferred.
- Must pass background check, TB test, and other pre-employment screening.
