About the Role

We are looking for a dedicated and caring Physical Therapist to provide high-quality patient care in the comfort of their own homes. As a Home Health Physical Therapist, you will use your clinical skills and expertise to develop and implement individualized care plans that promote independence, comfort, and dignity for our patients.

Responsibilities
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
  • Provide manual therapeutic exercises, gait training, balance, and other interventions to improve patient mobility and function
  • Collaborate with nursing and other healthcare professionals to ensure comprehensive patient care
  • Continuously assess and revise patient care plans to ensure optimal outcomes
Requirements

To be successful in this role, you will need:

  • A current and active license to practice as a Physical Therapist in the state of New Jersey
  • Previous home healthcare experience is welcomed, but not required
  • A calm and reassuring presence, active listening skills, and excellent assessment skills
  • Strong written and verbal communication skills
