BAYADA Home Health Care is seeking a compassionate and skilled Physical Therapist to join our team in providing high-quality care to patients in the comfort of their own homes.

About the Role

As a Home Health Physical Therapist, you will be responsible for assessing and treating patients with physical disabilities or injuries, helping them to regain mobility and independence. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Key Responsibilities:

  • Conduct thorough assessments of patients' physical abilities and develop personalized care plans to help them achieve their goals.
  • Provide manual therapeutic exercises, gait training, balance, and other interventions to help patients regain mobility and independence.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
  • Continuously assess and revise care plans to ensure optimal patient outcomes.
Requirements

To be successful in this role, you will need:

  • A current and active license to practice as a Physical Therapist in the state of New Jersey.
  • Previous experience in home healthcare is welcomed, but not required - we will provide comprehensive training.
  • Excellent assessment and communication skills, with the ability to work effectively with patients, families, and healthcare teams.
  • A calm and reassuring presence, with a passion for delivering high-quality patient care.
