About the Role

Creekside Health and Rehabilitation is seeking a dedicated and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our night shift team. As a nurse in our facility, you will play a vital role in providing high-quality care to our residents and making a meaningful difference in their lives.

Key Responsibilities
  • Provide daily care, health needs, safety, and comfort to residents, including assessments, documentation, and management of nursing units and staff.
  • Develop and implement resident care plans, maintaining up-to-date nursing care plans, documenting progress, and making recommendations for revisions.
  • Make daily and periodic resident visits to monitor and observe physical and mental conditions.
  • Perform routine nursing procedures, work closely with physicians and families to maintain health and mental state, and provide emotional support to residents and their families.
  • Document medical records, complete 24-hour reports, end-of-shift reports, shift narcotic counts, and document destruction of outdated/discontinued medications.
  • Maintain up-to-date resident medical records.
Requirements
  • Graduate of an accredited school of nursing and possess a valid RN/LPN license in good standing according to State and Federal requirements.
  • Thorough understanding of principles of safe and effective nursing practices within the State of Indiana.
  • Basic computer knowledge and skills.
  • Passion for caring for people.
  • Personal integrity and ability to deal tactfully with residents, family members, visitors, government agencies, and other personnel.
  • Ability to be discrete and protect the integrity of confidential information and stay within Corporate Compliance and HIPAA.
  • Knowledge of personal computers, Word, Excel, and working with office machines such as fax and copying machines.
