Physician - Primary Care Specialist

We are seeking a highly skilled and compassionate Primary Care Physician to join our team at CenterWell Senior Primary Care. As a key member of our care team, you will play a vital role in delivering high-quality, patient-centered care to our seniors.
Responsibilities- Evaluate and treat patients in accordance with standards of care, utilizing your expertise in geriatric medicine to provide comprehensive care.
- Collaborate with our interdisciplinary care team to coordinate patient services, including specialty referrals, hospital and SNF coordination, and durable medical equipment.
- Act as an active participant and key source of medical expertise with the care team through daily huddles, ensuring seamless communication and coordination of care.
- Assist in the coordination of patient services, including but not limited to specialty referrals, hospital and SNF coordination, durable medical equipment and home health care.
- Complete all medical record documentation in a timely manner, working with a quality-based coder to optimize coding specificity.
- Follow policy and protocol defined by Clinical Leadership, ensuring adherence to best practices and regulatory requirements.
- Meet with Regional Medical Director and Center Administrator to discuss quality of care, review of outcome data, policy, procedure and records issues.
- Participate in potential growth opportunities for new or existing services within the Center, driving innovation and excellence in patient care.
- Participate in the local primary care 'on-call' program of CenterWell as needed, ensuring seamless coverage and patient care.
- Assure personal compliance with licensing, certification, and accrediting bodies, maintaining the highest standards of professional conduct.
- Current and unrestricted medical license or willingness to obtain a medical license in the state of practice; eligible and willing to obtain licenses in other states in the region of assignment, as required.
- Graduate of an accredited MD or DO program of an accredited university.
- Excellent verbal and written communication skills, with the ability to effectively communicate with patients, families, and healthcare teams.
- Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients, fostering a culture of collaboration and respect.
- Fully engaged in the concept of 'Integrated team-based care' model, with a commitment to delivering high-quality, patient-centered care.
- Willingness and ability to learn/adapt to practice in a value-based care setting, with a focus on quality, safety, and patient satisfaction.
- Superior patient/customer service skills, with a commitment to delivering exceptional care and service to our patients and families.
- Basic computer skills, including email and EMR, with the ability to effectively utilize technology to support patient care and communication.
- Board Certification or Eligible to become certified (ABMS or AOA) in Family Medicine, Internal Medicine or Geriatric Medicine preferred.
- Active and unrestricted DEA license, with the ability to prescribe medications as needed.
- Medicare Provider Number, with experience in managing Medicare Advantage panels and understanding of Best Practice in coordinated care environments.
- Minimum of two to five years directly applicable experience, with a focus on geriatric care and value-based medicine.
- Knowledge of Medicare guidelines and coverage, with the ability to effectively navigate complex reimbursement systems.
- Bilingual or multilingual skills, with the ability to communicate effectively with patients and families from diverse backgrounds.
- Knowledge of HEDIS quality indicators, with a commitment to delivering high-quality, patient-centered care.
