Job Summary

We are seeking a highly skilled and experienced Pediatric Heart Transplant Coordinator to join our team at Ascension Seton Northwest. As a key member of our Cardiac Transplant Care Services team, you will be responsible for coordinating and managing the care of pediatric heart transplant patients.

Responsibilities
  • Develop and oversee program initiatives, making recommendations and acting as a resource for all nursing staff related to patient care and clinical standards, policies, and procedures.
  • Develop and document care plans and therapy programs, ensuring that necessary resources and equipment are obtained as ordered.
  • May supervise and direct nursing staff, providing guidance and support to ensure high-quality patient care.
  • Coordinate and assemble documentation as required, ensuring accurate and timely completion of tasks.
Requirements
  • Licensure/Certification/Registration: BLS Provider obtained within 1 month of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Registered Nurse credentialed from the Texas Board of Nursing required.
  • Education: Diploma from an accredited school/college of nursing or required professional licensure at time of hire.
  • Additional Preferences: 2+ years pediatric experience preferred, Cardiac experience preferred, Transplant experience.
