Job Title: Wound Care Registered Nurse

Vivo HealthStaff is seeking a highly skilled Wound Care Registered Nurse to join our team in San Marcos, California.

Job Summary:

The Wound Treatment Nurse will be responsible for coordinating and providing direct and indirect nursing care to patients at the center and in their homes. This role requires close collaboration with the Physician, RCFE facilities, clinic staff, and other interdisciplinary team members to manage ongoing care, complex clinical situations, and support the operation of the primary care clinic.

Key Responsibilities:
  • Conduct comprehensive nursing assessments to determine patient care needs, preferences, and goals, and actively participate in IDT meetings to develop patient care plans.
  • Visit patients' homes to deliver nursing interventions and maintain a healthy and safe environment.
  • Deliver and document nursing interventions as agreed upon in patient care plans, including administering medications, performing tests and treatments, and responding to physician orders.
  • Remain alert to pertinent input from other team members, patients, and caregivers, and promptly update the IDT of any changes in patients' condition or medical status.
  • Work with the Physician and other members of the IDT to manage smooth care transitions between settings.
  • Instruct patients, families, and caregivers regarding the disease process, self-care techniques, and prevention strategies.
  • Ensure the plan of care is communicated and adhered to by the contracted RCFEs where patients reside through regularly established joint conference meetings.
  • Provide or coordinate patient-related education to the contracted RCFEs.
  • Act as the liaison between the organization and contracted RCFE facilities to ensure all appropriate and essential services are implemented for patients.
  • Oversee the coordination of specialty appointments, day center visits, and other scheduling needs for patients residing in RCFEs.
  • Conduct regular visits to contracted RCFEs to facilitate care coordination.
  • Oversee all direct wound care activities to ensure consistent practices.
  • Identify, plan, implement, and evaluate skin care in patients.
  • Manage and treat specific skin disorders.
  • Coordinate weekly skin rounds.
  • Perform assessment evaluations using observation and inspection techniques.
  • Confer with the Interdisciplinary Team in the development of the care plan.
  • Provide treatment for skin disorders within established wound care protocols.
  • Monitor the use of preventive measures such as inspection, turning, and positioning.
  • Precept new nurses and demonstrate individual procedures to new nursing personnel.
  • Order and track the usage of supplies/equipment in prevention and treatment.
  • Assist in modifying the treatment regimen to meet the needs of the patient.
  • Provide information to patients and caregivers on prevention techniques and ways to maintain or improve their health and psychosocial status.
  • Participate in end-of-life care coordination and support.
  • Provide backup coverage to other nursing functions.
  • Complete all required documentation accurately and in a timely manner.
  • Protect privacy and maintain confidentiality of all company procedures and information about employees, patients, and families.
  • Follow policies and procedures and participate in any required Quality Improvement activities, staff training, and meetings.
  • Perform other duties as assigned.
Requirements:
  • Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred.
  • Wound care or treatment certification required, or to be obtained within 6 months of hire.
  • Unencumbered California Registered Nurse (RN) License.
  • Current Basic Life Support (BLS) / First Aid Certification required.
Preferred Qualifications:
  • Minimum of two years nursing experience in a clinical setting with a frail or elderly population.
  • Nursing knowledge and skills necessary to treat frail, elderly patients and manage complex clinical situations.
  • Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
  • Wound care or treatment certification.
  • Excellent clinical, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members.
  • Experience and competency working with people from diverse backgrounds.
