Job Summary:

Leoforce is seeking a skilled Private Duty Nurse LPN to join our team. As a Private Duty Nurse LPN, you will be responsible for providing high-quality patient care in a private setting.

Responsibilities:

  • Implement physician-prescribed plans of care for patients
  • Conduct patient assessments and coordinate care with healthcare teams
  • Administer prescribed medications, treatments, and therapies as ordered by physicians
  • Document patient care in a timely and compliant manner
  • Provide health education and training to patients and their families
  • Accept and write verbal orders from physicians as required, and obtain co-signatures from Case Managers
  • Follow up with patients, families, and healthcare teams on patient concerns and changes in condition
  • Perform other duties as assigned by management

Requirements:

  • Active, unencumbered LPN license in the state of practice
  • Current CPR certification from the American Red Cross or American Heart Association
  • TB skin test within the last 12 months
  • At least six months of prior hands-on nursing experience (preferred but not required)
  • Reliable transportation
