Physical Therapist Home Health - Weekender

VNA Health Group is seeking a skilled Physical Therapist to join our dynamic team in Independence, Ohio. As a Physical Therapist Home Health - Weekender, you will play a vital role in providing high-quality care to patients in their homes.

Responsibilities:
  • Conduct initial assessments and develop personalized plans of care for patients
  • Provide direct physical therapy services, including performance of physical therapy procedures and instruction of patients and caregivers
  • Collaborate with other healthcare professionals to ensure comprehensive care and make referrals as needed
  • Re-evaluate and revise plans of care as necessary with physician approval
  • Submit timely and accurate documentation
Qualifications:
  • Bachelor of Science Degree in Physical Therapy required, post-graduate degree preferred
  • One year experience in a supervised healthcare setting
  • Licensed, in good standing, to practice physical therapy in the State of Ohio
  • CPR certification required
  • Valid Driver's License
