Job Description:

  • Develop and implement individualized Integrated Service Plans (ISPs) for assigned clients.">
  • Collaborate with clinical staff to ensure timely completion of required paperwork and documentation.">
  • Facilitate Child & Family Team (CFT) Meetings to promote coordination among service providers.">
  • Establish and maintain effective relationships with community partners, such as schools, hospitals, and social services.">
  • Assist clients in navigating complex systems, including healthcare, education, and government agencies.">

Requirements:

  • ">
  • Bachelor's Degree in Social Work, Psychology, or related field required.">
  • 2+ years of experience in behavioral health case management or a related field.">
  • Level 1 Fingerprint Clearance Card without restrictions.">
  • AZ driver's license and reliable transportation.">
