Job Title: Registered Nurse Case Manager

We are seeking a compassionate and skilled Registered Nurse Case Manager to join our team at Conemaugh Hospice. As a key member of our hospice care team, you will provide end-of-life care to patients and their families, promoting physical, psychosocial, emotional, and spiritual well-being.

Responsibilities:
  • Develop and implement individualized plans of care for patients, collaborating with other healthcare professionals as needed.
  • Conduct regular assessments and evaluations to ensure high-quality patient care.
  • Provide education and support to patients and their families, addressing their physical, emotional, and spiritual needs.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
  • Current RN licensure in the state of practice.
  • One year of clinical experience, preferably in hospice or palliative care.
  • Current CPR certification.
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
