We are seeking a Patient Flow Specialist to join our team at St. John's Community Health.

About the Role:

  • The Patient Flow Specialist will be responsible for coordinating patient flow, including scheduling appointments and ensuring timely access to care.
  • This role requires excellent communication skills and the ability to work in a fast-paced environment.
  • The ideal candidate will have experience with electronic health records and practice management systems.

Responsibilities:

  • Coordinate patient flow, including scheduling appointments and ensuring timely access to care.
  • Work closely with clinical staff to ensure that patients receive timely and effective care.
  • Collect and maintain accurate records of patient interactions, including phone calls and in-person visits.
  • Perform routine clerical duties, such as filing and photocopying.

Requirements:

  • High school diploma or equivalent required; degree in healthcare or related field preferred.
  • Minimum 1 year of experience in a patient flow or coordination role, preferably in a community health setting.
  • Excellent communication and organizational skills.
  • Ability to maintain confidentiality and handle sensitive information.
  • Experience with electronic health records and practice management systems.

Salary Range: $48,000 - $58,000 per annum, depending on experience.
