Job Title: Hospice Nurse RN

Providence is seeking a compassionate and skilled Registered Nurse (RN) to join our team at Mother Joseph Care Center in Olympia, Washington.

About the Role:

We are looking for a dedicated and experienced RN to provide high-quality patient care in our skilled nursing unit. As a Hospice Nurse RN, you will be responsible for planning, organizing, and directing nursing services as part of the Interdisciplinary Team. You will work collaboratively with the team to administer individualized, comprehensive Plans of Care and help achieve unit goals.

Responsibilities:
  • Provide direct patient care, including assessment, planning, implementation, and evaluation of care
  • Collaborate with the Interdisciplinary Team to develop and implement individualized Plans of Care
  • Conduct regular patient assessments and updates to ensure accurate and effective care
  • Participate in unit-based quality improvement initiatives to enhance patient outcomes
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
Requirements:
  • Current RN licensure in the state of Washington
  • Minimum 1 year of experience in sub-acute care or hospice nursing
  • Strong communication and interpersonal skills
  • Ability to work collaboratively as part of a team
  • Basic computer skills and proficiency in electronic medical records
