Responsibilities:The Service Navigator will:
- Maintain regular communication with all family service providers to ensure seamless continuity of care.
- Conduct weekly home visits and more as needed.
- Schedule and participate in treatment reviews and other service provider meetings.
To be successful in this role, you will need:
- A minimum of one year direct experience working with children and families.
- A valid NYS Driver's License.
- The ability to demonstrate an understanding and appreciation of clinical concepts focusing on the strengths of the families you work with.
