Job Description

We are seeking a highly skilled and compassionate Medical Case Management Specialist to join our team at Proactive MD. As a key member of our clinical operations, you will play a crucial role in facilitating continuity in applicant and employee care, ensuring seamless transitions between client occupational health services and community provider networks.

Key Responsibilities

">

Your key responsibilities will include:

">
  • Coordinating candidate and employee care services to ensure seamless transitions and high-quality outcomes;
  • Ensuring medical records are provided to Proactive MD following visits and sending them to employers as required;
  • Collaborating with the Clinical Operations Manager and Client representative to develop employer Occupational Health case management plans;
  • Demonstrating exemplary customer service in coordination and communication surrounding Health Center visits;

Requirements

To succeed in this challenging yet rewarding role, you will need:

">
  • A minimum of 2 years of experience working in an Occupational healthcare setting or similar role;
  • Knowledge and understanding of OSHA medical surveillance requirements and process;
  • Current BLS certification through American Heart Association valid for at least 90 days after start date;
  • Strong communication and interpersonal skills, with the ability to build and maintain relationships with people from diverse backgrounds;
  • Ability to prioritize tasks and manage multiple projects simultaneously;

Estimated Salary Range: $65,000 - $75,000 per year
