Job Summary: In-Home Mental Health Counselor Needed for Vulnerable Populations

**About Us:** Stokes Counseling Services is a dynamic mental health private practice in Connecticut, committed to delivering high-quality care to our clients. We are seeking an experienced In-Home Mental Health Counselor to join our team and provide mental health services to seniors, individuals with medical conditions, or those with intellectual disabilities.

**Salary Range:** $75,000 - $95,000 per year, depending on experience.

**Responsibilities:**
* Assess the mental and behavioral health status of referred clients
* Conduct standardized counseling/talk therapy sessions for Stokes Counseling clients
* Perform intake/biopsychosocial assessments to evaluate clients' mental and behavioral health status
* Formulate differential diagnoses along with short-term and long-term goals
* Provide and maintain written documentation of intake, assessments, progress/contact notes, interventions, treatment plans, and termination notes within the designated time frame

**Requirements:**
* Active LPC licensure in Connecticut
* 1 - 3 years of experience conducting counseling/talk therapy sessions with diverse client populations
* Previous experience working in the medical field with seniors and/or intellectually disabled clients
* Passion for in-home work
* Ability to work collaboratively in a team-oriented environment
* Advanced proficiency in conducting intake assessments and formulating treatment plans
* Strong written communication and interpersonal skills
* Valid driver's license and access to a properly insured vehicle for work or reliable transportation
* Spanish speaking preferred (not required)
