Position Overview:

The Patient Care Assistant is tasked with managing patient interactions, including scheduling appointments and addressing inquiries. This role also involves utilizing data to identify and address care gaps, assisting patients in achieving their health objectives, and coordinating necessary resources to mitigate socioeconomic challenges.

Responsibilities include receiving and prioritizing requests from caregivers, documenting interactions, and actively resolving issues to enhance patient satisfaction.

This position collaborates closely with the Care Coordination Supervisor, Care Coordination Manager, and various healthcare professionals to ensure seamless patient care.


Work Schedule:
Day shifts

Key Responsibilities:


- Respond to patient needs by assisting with daily activities, monitoring vital signs, and effectively communicating with the healthcare team.


- Conduct 12-lead EKGs as per training and competency standards.

- Administer and document restraints as instructed by nursing staff or physicians.

- Provide wound care, stoma care, and collect specimens as required.

- Manage equipment and tube care, including JPs, hemovacs, nasogastric tubes, and Foley catheters.

- Assist with patient admissions, transfers, and discharges while adhering to safe handling protocols.

- Maintain inventory of office and patient care supplies.

- Perform unit secretary duties, including managing patient charts, responding to calls, and maintaining daily assignments.

- Support physicians and registered nurses as directed.

- Uphold the mission and values of Intermountain Health while adhering to service behavior standards.

- Follow infection prevention protocols and perform additional duties as assigned.

- May be required to assist in other departments based on operational needs.

- Availability for on-call duties during scheduled shifts may be necessary.

Qualifications:

Required:
- Completion of an accredited nursing assistant training program.
- Certification as a Nursing Assistant (CNA) in the relevant state.
- Current Basic Life Support (BLS) certification from the American Heart Association upon hire or within a specified timeframe.

Preferred:
- At least one year of experience in a healthcare environment.
