Position Overview:
We are seeking a Full-Time Licensed Practical Nurse (LPN) to join our dedicated team. This role is crucial in delivering high-quality nursing care to individuals with brain injuries, developmental disabilities, and other complex medical conditions.
Key Responsibilities:
As a Practical Nursing Specialist, you will:
- Monitor and assess the health status of individuals, ensuring the delivery of quality care.
- Administer prescribed medications and maintain accurate documentation.
- Implement medical treatments as directed by physicians and provide personal care services.
- Conduct health assessments and develop Individual Service Plans (ISP) tailored to each individual’s needs.
- Train and support staff in essential care practices, including infection control and medication administration.
- Act as a liaison between individuals served and other healthcare professionals to coordinate comprehensive care.
- Provide educational support to individuals on topics such as self-medication and health management.
Qualifications:
To be considered for this role, you should possess:
- An Associate's Degree in Nursing or a related field, or a state certification.
- A minimum of one year of nursing experience with the population served.
- Current state licensure as an LPN or LVN.
- A valid driver's license and proof of auto insurance.
- Additional certifications as required by state regulations, including CPR/FA and HIV/AIDS training.
- Strong communication skills to effectively interact with individuals, families, and team members.
