About the Role:

We are seeking a skilled Healthcare Specialist to join our team at Meridian Health Services. As a Healthcare Specialist, you will play a vital role in providing skills and development training to patients and families, helping them achieve developmental milestones and reduce psychosocial barriers.

Key Responsibilities:

  • Assist therapists in developing, implementing, and revising individual treatment plans.
  • Provide individual and group skills training and development to assist patients and families in developing skills such as self-care, daily life management, interpersonal communication, and problem-solving skills.
  • Work collaboratively with the clinical team to ensure coordination of integrated care for patients, including identification of health issues, compliance with treatment, and effective patient-to-medical-provider communication.
  • Complete CANS/ANSA re-assessments and provide consultation to staff on specific cases involving coordination of care.
  • Maintain positive working relationships with staff of other agencies and institutions, and act as a liaison between patients and agencies and/or institutions.

Requirements:

  • High school diploma or equivalent with 2 years of paid behavioral healthcare experience or equivalent college coursework toward a human services or health-related degree.
  • Associate or Bachelor's degree preferred.
  • A valid driver's license with a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance.
