Job Summary

We are seeking an experienced and passionate Care Team Specialist to join our growing organization. As a key member of our interdisciplinary team, you will work closely with our Center Director to ensure our participants receive the highest quality care. Your leadership and experience will play a crucial role in facilitating Interdisciplinary Team Meetings, ensuring we meet all regulatory requirements, and fostering a collaborative, accountable environment that truly aligns with each participant's values, goals, and preferences.

Key Responsibilities
  1. Lead and facilitate Care Team Meetings, morning huddles, and care planning discussions to ensure accurate capture of important details in our Electronic Medical Record (EMR).
  2. Create and maintain efficient workflows for care planning, documentation, and compliance, aligning with regulatory requirements and our high standards of care.
  3. Serve as a go-to expert in our EMR system, providing ongoing support and guidance to team members, helping them feel confident and capable.
  4. Monitor and ensure timely completion of assessments, care plans, service determination requests, and other IDT-related tasks.
  5. Partner with our Quality Improvement and Compliance team to audit care plans and ensure we meet regulatory requirements with excellence.
Requirements
  • Bachelor's Degree in a relevant field (e.g., nursing, social work, healthcare administration), Masters in Social Work or Healthcare Administration strongly preferred.
  • Compliance-focused mindset and familiarity with regulatory requirements related to PACE.
  • Proficiency in Microsoft Word, Excel, and PowerPoint.
  • Experience using Electronic Medical Records (EMR) systems.
