We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in the Reynoldsburg, OH area. As a LPN Homecare Specialist, you will have the opportunity to work with pediatric patients in a home setting and provide high-quality care and support.

About the Role
  • Utilize the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services.
  • Initiate the plan of care when directed and request revisions as patient status and needs change.
  • Administer medications and treatments as prescribed, and monitor and document effectiveness of intervention.
  • Review plan of care and program protocols, and identify areas for healthcare teaching/education, care coordination, goal setting, and discharge planning needs.
Requirements
  • Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) License for the state in which the nurse practices.
  • Current Health Certificate.
  • Current PPD or Chest X-Ray.
  • Current BLS card.
