Job Summary

We are seeking a highly skilled and compassionate Care Manager to join our team at HomeWell Care Services. As a Care Manager, you will be responsible for coordinating, implementing, and supervising the delivery of services to our agency clients in alignment with our mission and standards of care.

Key Responsibilities
  • Develop and implement written care plans for each client, including client functional limitations, nutritional needs, home medical equipment, and type and schedule of services.
  • Collect fees for initial services and provide caregiver introductions, reviewing care plans with clients and caregivers, and setting expectations for service delivery.
  • Supervise client care provided by agency personnel, ensuring caregivers comply with care plans, implement proper infection control practices, and observe and report changes in client service needs or condition.
  • Regularly communicate with clients and families to follow up on service delivery and ensure client satisfaction.
  • Participate in coordination of services with other community service providers and serve as a resource for clients and families, facilitating client education, empowerment, and quality of life.
Requirements
  • Two years of experience in home care, home health, or a related field.
  • A reliable means of transportation with current insurance and a valid driver's license.
  • Excellent written and verbal communication skills, with the ability to interact effectively with diverse audiences.
  • Impeccable follow-up and attention to detail, with the ability to exercise initiative, problem-solve, and apply sound judgment.
  • Able to supervise, assess, and evaluate care providers, and advocate on behalf of clients and families.
Compensation

$48.00 per hour
