About the Role

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our Psychiatry team at Signature Health. As an LPN, you will play a vital role in providing exceptional nursing support to our clients, working closely with other providers to ensure the best possible care.

Key Responsibilities
  • Consult and collaborate with staff and providers to execute appropriate plans of care.
  • Conduct patient intake, including vital signs, health history documentation, and preparation for examination and/or procedures.
  • Review no-show charts and determine follow-up actions.
  • Respond to patient telephone messages and document outcomes in patient records.
  • Administer medications safely, including injections.
  • Arrange and follow procedures for specimen collection, reviewing labs/test results, and contacting providers as needed.
  • Complete prior authorizations and patient assistance forms.
  • Review patient medications, update medication lists, and add prescription information.
  • Complete documentation in medical records and comply with state, federal, and departmental policies.
  • Coordinate patient care with outside healthcare facilities or emergency rooms when necessary.
  • Exhibit a non-judgmental, empathetic, and supportive approach when communicating with patients, families, and staff.
Requirements
  • Completion of an approved program in practical nursing.
  • Valid unencumbered LPN license in the state of Ohio.
  • 1+ years of recent LPN experience with high clinical expertise.
  • Experience in outpatient clinics or psychiatry preferred.
  • American Heart Association (AHA) Basic Life Support (BLS) certification.
  • Working knowledge of legal regulations and best practices in healthcare.
  • Excellent organizational and problem-solving skills.
  • Proficient with technology and electronic medical records.
Working Conditions
  • Work is typically performed in a standard interior/office/clinical environment.
  • Flexibility to work evenings and extended hours may be required.
  • Requires periods of sitting, standing, telephone, and computer work.
  • Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.
  • Possible exposure to blood-borne pathogens.
