Job Title: Private Duty LPN

Job Summary:

Aveanna is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to provide high-quality patient care in the comfort of their own homes. As a Private Duty LPN, you will work closely with our team to deliver personalized care that meets the unique needs of each patient.

Responsibilities:

  • Provide skilled nursing care to patients in their homes, under the supervision of a Registered Nurse.
  • Develop and implement individualized care plans that address the patient's physical, emotional, and social needs.
  • Monitor patient condition and report changes to the supervisor and/or physician as appropriate.
  • Document all activities, assessments, and nursing actions in a timely and accurate manner.
  • Participate in ongoing education and training to maintain knowledge of best practices and industry developments.

Requirements:

  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.

Preferred Qualifications:

  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.

Physical Requirements:

  • Must be able to speak, write, read, and understand English.
  • Must be able to travel.
  • Must be able to lift 50 pounds.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
  • Prolonged walking, standing, bending, kneeling, reaching, twisting.
  • Must be able to sit and climb stairs.
  • Must have visual and hearing acuity.
  • Must have strong sense of smell and touch.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
  • Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport.

Environment:

  • Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
