Position Overview:
We are in search of a skilled Care Team Leader to supervise and guarantee the effective execution of all services rendered to our clients in their designated regions. The individual will be tasked with ensuring that client care strategies align with client requirements and that caregivers deliver an outstanding level of service that reflects the vision, mission, and values of our organization.
Key Responsibilities:
- Maintain organized files for assigned clients, employee records, and all pertinent documentation
- Deliver direct training to field personnel regarding client Care Plans
- Oversee the performance of field staff who provide personal care to clients, ensuring quality service
- Effectively communicate schedules and updates to clients, field staff, referral sources, team leaders, managers, and the branch manager
- Prepare and submit routine departmental reports as necessary
- Document and maintain accurate records of client conditions and overall service delivery
- Conduct and document client satisfaction surveys, in-home visits, employee evaluations, and training sessions
- Address client complaints and issues appropriately; document and report significant matters for further attention or resolution as needed
- Ensure adherence to local, state, and federal regulations as well as company policies and procedures
- Perform additional job-related tasks as assigned
Qualifications and Educational Background:
- Bachelor's degree is preferred, though not mandatory
- Fluency in English and Spanish is required
- Management or supervisory experience of 2 years is preferred, but not essential
- Must be at least 18 years of age
- A high school diploma or equivalent is required
- Ability to thrive in a fast-paced environment with strong written and verbal communication skills
