About the Role

We are seeking a dedicated and compassionate Care Manager to join our team at Sunrise Senior Living. As a Care Manager, you will play a vital role in providing exceptional care and services to our residents and their families.

Key Responsibilities
  • Build Meaningful Relationships: Develop strong relationships with a designated group of residents and their families, providing assistance with activities of daily living and attending to individual care needs.
  • Notify Management of Changes: Inform management of any changes in a resident's condition and recommend adjustments in care and service as needed.
  • Assist with Life Skills: Assist residents with life skills and other life-enriching activities as indicated on their individual profile.
  • Participate in Care Planning: Participate in the development of Individualized Service Plans (ISPs) and monthly updates.
  • Ensure Resident Safety: Ensure the safety and well-being of residents during the shift, knowing their whereabouts and physically checking on them as needed.
  • Partner with Community Team: Collaborate with the community team to ensure compliance with OSHA requirements and promote Risk Management programs and policies.
  • Provide Excellent Customer Service: Respond to the dining needs of residents and guests while maintaining Sunrise hospitality and service standards.
Qualifications
  • Dedication to Serving Seniors: Demonstrate a passion for serving seniors with excellent customer service skills.
  • Positive Attitude and Flexibility: Possess a positive attitude and the flexibility to perform various duties in service to residents and the ability to work in a team environment.
  • Education and Certifications: High School diploma/GED accepted and may be required per state regulations. In states where appropriate, must maintain certifications.
  • Previous Experience: Previous experience working with seniors preferred.
  • Decision-Making and Problem-Solving: Possess the ability to make choices, decisions, and act in the best interest of residents.
