Position Overview

As a vital member of our healthcare team, the Registered Nurse - Case Manager plays a crucial role in coordinating comprehensive patient care. This position is designed for individuals who are dedicated to enhancing patient outcomes through effective management of care plans.

Department Details
  • Department: Case Management - Emergency Department
  • Schedule: 4x10 with Rotating weekends - 1:00pm - 11:00pm
  • Hospital: Ascension Sacred Heart Hospital - Bay Medical

Key Responsibilities
  • Develop and implement an interdisciplinary care plan for patients, ensuring a seamless transition from admission to discharge.
  • Assess and monitor patient care through thorough evaluations and documentation.
  • Advocate for patients by identifying and addressing barriers to care.
  • Maintain effective communication with supervisors and healthcare providers to ensure the successful execution of care plans.
  • Serve as a resource for accurate medical record documentation and treatment options for post-discharge care.

Qualifications
  • Licensure: Must possess a valid Registered Nurse license from the Florida Board of Nursing prior to hire.
  • Certification: BLS Provider certification preferred, with acceptance from the American Heart Association or American Red Cross.
  • Education: Diploma from an accredited nursing school or professional licensure at the time of hire.

Preferred Experience
  • Minimum of 1 year of experience in case management and discharge planning.
