We are seeking a dedicated and compassionate Home Health Aide to join our team. As a Home Health Aide, you will provide essential care and support to patients in their own residences, promoting their independence and dignity.

Responsibilities

- Provide basic care and assistance with daily activities
- Monitor patients' conditions and report any changes to the nursing team
- Collect and record vital information for patient care and treatment

Requirements

- High School diploma or equivalent preferred
- Completion of an approved Home Health Aide training program
- Minimum one year experience as a Nurse Aide or Home Health Aide preferred
- Ability to read, follow written instructions, and document care given
