About the Role

We are seeking a highly skilled and compassionate Senior Home Health RN - CWOCN to join our team at Providence Home and Community Care. As a Senior Home Health RN, you will play a vital role in providing high-quality care to patients in their homes, under the direction of a physician.

Key Responsibilities
  • Conduct thorough nursing assessments of patients and develop individualized care plans to meet their unique needs.
  • Collaborate with other healthcare professionals to coordinate patient care activities and ensure seamless transitions between care settings.
  • Provide education and support to patients and their families to promote health and well-being.
  • Participate in quality improvement initiatives to enhance the delivery of care and services.
Requirements
  • Bachelor's Degree in Nursing
  • Washington Registered Nurse License
  • National Provider BLS - American Heart Association
  • National Provider Identifier (NPI) number
  • Certified Wound Ostomy Continence Nurse (CWOCN)
  • 3 years of Nursing experience
Preferred Qualifications
  • Hospice, Homecare, Long Term Care, medical/surgical, and/or oncology experience
