Job Summary:

The Advanced Healthcare Provider is responsible for delivering high-quality patient care, including education, medical management, and collaboration with healthcare teams. This role focuses on health maintenance and disease management, ensuring continuity of care through effective communication and interventions.

Key Responsibilities:
  1. Provide patients and their families with comprehensive education based on their condition, identifying needs to optimize understanding.
  2. Obtain comprehensive medical histories and perform physical examinations/assessments, initiating admission orders as necessary.
  3. Provide medical management and interventions for acute and chronic health problems.
  4. Implement a plan of care and perform procedures in accordance with the Advanced Practitioner's education, training, experience, and competencies.
  5. Refer patients to specialists and/or therapists as necessary and in a timely manner to ensure continuity of care.
  6. Order and interpret lab studies and diagnostic tests to evaluate treatment effectiveness, notifying patients promptly of test results.
  7. Collaborate with physicians to verify diagnoses, identify high-risk situations, and address changes in the patient's status.
  8. Write orders for prescription medications under established protocols.
  9. Serve as a liaison and resource for physicians, healthcare team members, patients, and families.
  10. Participate in health promotion, prevention, and maintenance initiatives for the Altru Health System, community, and region.
  11. Perform other duties as assigned to meet the needs of the department/organization.
Patient/Caseload Information:
  1. Primarily adult patients, with some adolescents; approximately 12 patients per day.
Staff Information:
  1. Collaborates with psychiatrists (available 24/7), nurses, psych techs, social workers, and discharge planners.
