As a Care Coordinator with Dartmouth Health, you will play a vital role in ensuring seamless transitions between healthcare services, facilitating effective communication among healthcare professionals, and promoting patient-centered care.
Key Responsibilities:
- Collaborate with healthcare teams to coordinate patient care and transitions
- Promote patient and family-centered approaches to care coordination and transitions
- Facilitate connections between patients, providers, and caregivers to community resources
- Organize patient care activities to optimize healthcare resource utilization
Requirements:
- Bachelor's degree in nursing required; Master's preferred
- Licensed as a Registered Nurse in the State of New Hampshire
- BCLS certification required within 30 days of hire
- Board Certification in Case Management (CCM or CMC) preferred
- Five years experience in a healthcare setting; experience as a Case Manager is an asset
