Key Responsibilities
  • Participate in ongoing interdisciplinary assessment of patients to ensure comprehensive care.
  • Develop and evaluate patient care plans in collaboration with healthcare teams.
  • Provide ordered services by physicians and recognize/report changes in patient condition.
  • Counsel and educate patients and families on healthcare-related needs.
  • Document patient progress and complete timely and accurate clinical notes.
  • Assist RNs and physicians in performing specialized procedures and duties.
  • Participate in Quality Assessment and Performance Improvement (QAPI) program.
  • Access protected health information (PHI) necessary to fulfill duties and responsibilities.
Requirements
  • Graduate of a practical (vocational) nursing program and current licensure as a LPN/LVN in the state(s) of practice.
  • At least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program.
  • Able to perform and provide evidence of current CPR certification for adults, children, and infants.
  • Able to effectively communicate in English with all levels of the workforce.
  • Able to prioritize multiple functions or tasks and read/interpret technical instructions.
Working Conditions

Able to constantly travel within the geographic area serviced by the office from assignment to assignment. Frequent exposure to communicable diseases, bloodborne pathogens, and/or other potentially infectious or hazardous materials and situations that require following extensive safety precautions.
