Nurse Case Manager

ChenMed is seeking a highly skilled and compassionate Registered Nurse (RN) to serve as a Clinical Lead for our Community Care team. As a key member of our team, you will play a vital role in stabilizing our highest-risk patients, coordinating care transitions, and providing clinical supervision to other team members.
Key Responsibilities- Provide in-home and telephonic visits to patients at high-risk for hospital admission and readmission, with the goal of preventing unnecessary hospitalizations.
- Conduct initial assessments and develop comprehensive care plans for patients, working closely with Licensed Practical Nurses (LPNs) to ensure seamless care delivery.
- Supervise LPNs during patient visits, providing education and support as needed, and ensuring proper discharge from case management.
- Perform clinical and social determinants of health (SDoH) assessments, including disease-oriented assessments, medication monitoring, health education, and self-care instructions.
- Coordinate care plans, ensuring individualized care reflects patient needs and available community resources.
- Communicate effectively with patients, families, and healthcare providers to promote patient engagement and self-management.
- Monitor patient progress, adjusting care plans as needed, and ensuring timely discharge from case management.
- Collaborate with multidisciplinary teams to address patient needs, coordinate services, and ensure seamless care delivery.
- Stay up-to-date with industry developments, best practices, and regulatory requirements, applying this knowledge to improve patient outcomes and care delivery.
- Associate degree in Nursing required; Bachelor's degree in Nursing (BSN) or RN with a related clinical field preferred.
- Valid, active Registered Nurse (RN) license in the state of employment required.
- Minimum 2 years' clinical work experience required; 1 year of case management experience in community care highly desired.
- Certified Case Manager certification preferred; certification through the Commission for Case Manager Certification (CCMC) or the American Association of Managed Care Nurses (CMCN) desired.
