Job Summary

Regency Integrated Health Services is seeking a highly skilled Nurse Practitioner to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and coordination services to our patients.

Key Responsibilities
  • Provide Provider-level care coordination for all patients, ensuring timely and effective delivery of healthcare services.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans.
  • Provide education and support to patients and their families to promote healthy behaviors and disease management.
  • Participate in quality improvement initiatives to enhance patient care and services.
  • Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements.
Requirements
  • Master's degree in Nursing or related field.
  • Current licensure as a Registered Nurse and Advanced Practice Registered Nurse in the state of Texas.
  • Board certification in a relevant specialty.
  • Minimum of 2 years of experience in a similar role.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
