Position Summary:
St. Patrick Center, part of Catholic Charities, is seeking a dedicated Case Manager for Project LIVE. This role focuses on delivering comprehensive client support within our Permanent Supportive Housing Program.
The Case Manager will provide housing-centered case management services and advocacy for individuals experiencing chronic homelessness, along with various other responsibilities.
As a vital member of the Project LIVE team, the Case Manager will be tasked with developing individualized treatment plans, collaborating with program leadership, and ensuring stability for clients. Experience working with homeless populations and conducting home visits is essential.
Working Hours and Compensation:
The standard working hours are Monday to Friday, from 8 am to 4:30 pm. The salary range for this position is between $40,000 and $50,000.
